Medical records can be obtained by you, or forwarded, by sending a written request, with a $10.00 fee to cover costs, to:

Include your date of birth, request for records and signature.  If you have kept my visit summary sheets and lab results (provided to you at every visit) you already have what you need.  I will not forward any records received from other practices (this is routine medical records practice).
